Safety considerations regarding noninvasive positive pressure ventilation following esophagectomy.

چکیده

I read with great interest the article by Raman et al 1 in a recent issue of CHEST (February 2015) that evaluated the pressure tolerance of esophagectomy anastomosis ex vivo and in vivo in a pig model in comparison with esophageal pressure during noninvasive positivepressure ventilation (NPPV). Th e authors showed that anastomosis can tolerate higher pressures than those transmitted to the esophagus during NPPV, thus demonstrating the safety of NPPV after esophagectomy. 1 Despite these data, however, in the clinical setting caution is necessary for several reasons when using pressures . 20 cm H 2 O during NPPV after esophagectomy.

Noninvasive positive pressure ventilation for acute respiratory failure following oesophagectomy: Is it safe? A systematic review of the literature.

OBJECTIVE To find, critically appraise and synthesise all published studies so as to determine the safety and spectrum of use of noninvasive positive pressure ventilation for acute respiratory failure following oesophagectomy. DESIGN Systematic review. METHODS The MEDLINE and EMBASE databases were searched and the quality of the studies and any bias or confounding were rated according to es...

Comparison of direct and indirect blood pressure in patients under esophagectomy surgery: a prospective observational study

Background: Arterial pressure is one of the most important physiological variables and often needs to be monitored repeatedly or continuously in perioperative period. Arterial pressure monitoring is one of the standard monitoring in operating room. During general anesthesia, blood pressure can be measured by using a noninvasive arterial pressure method or continuous invasive arterial pressure b...

Noninvasive Ventilation using Bipap: Expanding Indications to Post Cardiac Surgery Care

6 In the previous issue of Annals, a single‐center unblinded study assessing the outcomes of noninvasive bilevel positive airway pressure ventilation versus typical positive pressure ventilation through an endotracheal tube for patients with respiratory failure following initial extubation after open heart surgery provides an important reminder that traditional care requires continuous question...

Early Effectiveness of Noninvasive Positive Pressure Ventilation on Right Ventricular Function in Chronic Obstructive Pulmonary Disease Subjects with Acute Hypercapnic Respiratory Failure

Introduction: Noninvasive positive pressure ventilation (NIPPV) has become an integral tool in the management of acute hypercapnic respiratory failure (AHRF) in chronic obstructive pulmonary disease (COPD). This study was performed to evaluate the early effects of NIPPV on pulmonary artery pressure (PAP), serum N-terminal pro BNP (NT-proBNP), and ventilatory parameters in the COPD patients with...

Efficacy and safety of artificial pneumothorax under two-lung ventilation in thoracoscopic esophagectomy for esophageal cancer in the prone position.

BACKGROUND Thoracoscopic esophagectomy for esophageal cancer performed using two-lung ventilation in the prone position has many advantages, such as convenient anesthesia induction and maintenance, and good oxygenation. We examined the safety of surgery and anesthetic management by following chronological changes in intraoperative respiration and hemodynamics. METHODS We focused on the most r...
